What are the safest batteries to use?
Are any kind safe to stack?
Whats the safest way to make a high voltage mod?

protected batt are safe, PCB protected (trustfire,ultrafire..), or chemistry protected. It`s safe to stack, only tube have some construction specification for safety.

For the last question : make a clean work, good soldering, without short circuit, and build some electronic protection or use protected IC.

Easiest to build and safest higher voltage mod would be two protected cells in series driving a regulator, ideally a switching regulator. I recommend against stacking the 3.2V cells to avoid using a regulator. That's the least safe configuration.
