From the sources above, I would venture to guess they melted down an unfilled clearo to get those levels. The other devices contained 6 times less formaldehyde than cigarettes. I think they were all prefilled carto cigalike type devices. So, when they tested the "high power new-generation" device, I would bet they did not add any eliquid just to get the worst numbers possible.
Side note: the use of a standard smoking machine may seriously skew vaping results. The draw is totally different. An experienced user can use draw pressure to control coil temperature.
