I'm sure somebody will come around with a more detailed explanation, but here's the quick and dirty:

Higher PG generally results in a thinner liquid and usually with thinner/lighter vapor production. As you increase the VG ratio, you get a thicker liquid but also thicker/more voluminous vapor production. The thicker the liquid, though, the more prone to gunking up your atomizer head you are.

People tend to find a "sweet spot" ratio that works best for them - for me, it's about 30% VG. You still get great vapor production and I've never noticed my stuff to clog or gunk up too incredibly quickly.

PG = Propylene Glycol. PG is thinner than VG and acts as a flavor carrier and promotes throat hit.

VG = Vegetable Glycerin. VG is thick. It is also naturally sweet. VG tends to diminish flavor and throat hit slightly, but it produces very thick vapor.

Apart from sensitivity to one or the other, most people find a balance between the two that gives them the desired levels of flavor, throat hit and vapor. Because of the different viscosity, the ratio also plays a part in how well a juice wicks, or flows into the heating coil. There is no right or wrong, just what works best for the individual.

Distilled water is also an ingredient used in ejuice, typically in high VG juice, and is mainly used to thin out a juice enough so that it wicks easily enough.

as a carrier for nebulized medicine

Thanks for posting that! About 5 years ago I was suffering pretty bad from asthma and you posting that reminded me that the nebulizer used to irritate my chest when I would use it in a similar way that e-cigs have been irritating me. So, I'm thinking the PG is what has been doing it. My brother just sent me an EVOD to try since I couldn't use the little ecigs I used last time I quit smoking without coughing like crazy. Also read a posting on here explaining about better ways to use the ecigs. So glad my brother told me to get on this forum. :D
